With reference to the periodic conversations that we have had with you, this is a written documentation of the same.
It has been observed that (incident) which is strictly against the office code of conduct.
The organization is compelled to give you a final warning any repeat incident/s will attract strict disciplinary action leading to and including termination of services.

Sasmita, i do agree with you to a great extent. But there is one factor that you need to look at in our line of career is the expanse of industries available for us to explore. HR is the only function that has more or less similar work requirements irrespective of the industry that we work in.

You will need to first discuss the issue with both the individuals involved and understand their part of the story. If you feel that the lady is right then you have an option to either issue a warning letter against the culprit or sack the person. If you sack the person you will have to quote the clause in his appointment letter which mentions "indiscipline" as a violation of the company policies.
Try to be discreet about the whole issue as it involves a lady, who might be very uncomfortable facing peers and colleagues as a result of embarrassment.
