Appreciate your interest of sharing this questionnaire. However, I think there can be many more points/dimensions to capture in a questionnaire (designed with a motive to know the pulse of interviewees about the overall recruitment).

Moreover, few points/items of the questionnaire sound redundant. And, some points should be left to organization to organization because company policy may or may not welcome those items (e.g, relocation expenses). Plus, to those who are not in HR area, recruitment process means interview rounds or vice versa. So, questionnaire items should be free from double meaning and chance of misinterpretation.
