pradeep intersting stuff but i beg to defer from what you said.
i would like to emphasize the fact the prospective employee who is joining our organizations should have an excellent experience during and in the process of recruitment.
also recruitment is not a cost instead its an investment for an organization to recruit best talent from the market. also we need to provide the best experience if you want the best persons to join your organization. organization is as good as the people you have and you would be a mediocre if you employ mediocres

there is no one size fits all type of questionnaire
you need to understand whats important for your organization both technical, aptitude and other skill set requirements and accordingly work with appropriate folks in your org and come up with a questionnaire

Couple things that come to my mind
1. Put folks into jobs that they fit the most, based upon their skill set and experience, and they aspire. You got to discuss individually and finalize it.
2. Challenge them : Folks do enjoy challenges. Challenge them to their core. Make them commit to the deliverables
3. Set intermediate motivational reviews
4. Track progress

this is an excellent example that we teach in analyze phase of six sigma..not to mention that is was true and a real example
