Hr Department will provide the PF numbers as per continuity. The declearation form should submit to Regional PF office. Before submitting the details just Verify all the details correct it will be used at the end of Claim requestion

From the gross salary ESI should deducte and total ESI Contribution is 6.5%. In this Employee need to contribute 1.75% from this Gross Salary and Employer should Contribute 4.75%. And it limits to 15000 of Gross Salary. According to ESI Act its mandatory.
