KRA Vs Competencies

How many of you received KRA's and got formulated with your respective competencies at your comp? we have a 3 x 3 grid which states the develpmental needs for an individual. why most of the companies fail to go indept of this context? finally this reflects performance of an individual during appraisals. who should be blamed on the same. its HR / Management?
this is just to understand different versions from all my HR friends.
