360 Degree Performance Appriasal - Format

Thanks for posting these formats, they are for sure useful,
Infact, very recently I was planning to implement this at our office for all the TMs.
Well, 360 degree appraisal, has its own positive & negative points...isnt it?
Before putting it to practice we need to educate & give a good overview about the details of the forms, purpose of this appraisal...just to avoid the bias feedbacks...
There would be lots of mixed feelings from peers, subordinates & supervisors, which makes it a difficult task for a healthy judgement...
